FREQUENTLY ASKED QUESTIONS

  • Relish Health is dedicated to filling the void found in our healthcare system by shifting the focus from insurance corporation demands to a meaningful relationship with each patient. The physicians at Relish Health are not participating providers in any insurance plans; therefore, the office does not accept or bill insurance. Relish Health will provide a detailed receipt (also called a “superbill”) for the services performed and you can submit this to your insurance carrier for possible reimbursement. Many patients find their policy provides for coverage with an out-of-network provider. It is your responsibility to check with your insurance company to review the coverage rules, requirements, and policies. Important exceptions are Medicaid and Medicare. Relish Health’s services cannot be billed to either of these programs.

  • Dr Leazenby is a Functional Medicine consultant for adults and adolescents. She does not provide the screening tests (PAP, mammogram, etc.), well visits, vaccinations, office procedures and other annual services that your primary doctor provides.

  • No, labs are not drawn at the time of your visit. Your Relish Health physician will give you a lab order to take to a facility that is inside your insurance network and close to your home. Many lab specimens can be collected at home.

  • Due to the length of each visit, Relish Health physicians see a limited number of patients in a day. Relish Health has found it necessary to implement a “last minute” cancellation and no-show charge of $75. New patients will forfeit their deposit and may be asked to prepay their rescheduled new patient visit.

    Relish Health appreciates your understanding with keeping your appointments or informing the office within a reasonable time if you are unable to keep the appointment. This reduces reschedules and allows others to benefit from the known availability.

  • Your Relish Health physician is happy to answer brief questions you may have about your established treatment plan via the secure patient portal system at no charge. If it is a complex question or new medical issue, your physician will recommend scheduling a 15 minute consultation (or longer if needed). Please note, your Relish Health provider does not share protected health information via email. Due to privacy laws, your provider must communicate via secure portal versus email.

    Your provider does their best to respond within 3 business days with the exceptions of holidays. If you are experiencing a medical emergency you should call 911 or contact your primary care practitioner.

    Phone conversations are billed as Telehealth visits.

  • The Relish Health fees and are available here. Cash, check, MasterCard, Visa, American Express, Discover, and your HSA (health savings account) or FSA (flexible spending account) cards are all accepted.

  • Once you have registered with the portal and selected an appointment, you can begin the process of completing your intake forms online and submitting them through the patient portal.

    Please note, you can upload any prior medical records (including labs, procedure reports, etc.) to the portal if you would like your provider to review the data prior to your appointment. Bring all medications, supplements, shakes/nutritional products with you to your consultation. Spouses/partners are welcomed and encouraged.
